Ankle arthritis is less common than knee or hip arthritis, and it most often follows an old injury such as a fracture or repeated sprains. The cartilage wears away, causing pain, stiffness and swelling.
First steps
- Walking aids, supportive footwear or a brace.
- Weight control, activity changes and anti-inflammatory measures.
- Injections in selected cases.
- Ankle arthroscopy to clean the joint if there is impingement or loose tissue, before the cartilage is completely worn.
Realignment
If the deformity is in the lower leg bone, a supramalleolar osteotomy can shift the load to healthier cartilage and delay the need for a more major operation.
When the joint is worn out
- Ankle fusion (arthrodesis): the two bones are joined so that the painful joint no longer moves. It gives a durable, strong result, and it can be done through a lateral or an anterior approach.
- Ankle replacement: the worn surfaces are replaced with an implant, keeping movement at the ankle. It is chosen for suitable patients, depending on age, bone quality, alignment and activity.
Neither is better for everybody. The decision is made together after examination, imaging and a discussion about your daily life and expectations.
